FOR HEALTHCARE PROFESSIONALS AND ADMINISTRATORS:

Campus Care by Providence At Home

|   Supporting your staff, so your patients can
continue to get the best care

Providence At Home is a subsidiary of Providence Life Services, which has been serving people’s residential, medical, and social needs for decades. We are state-licensed, Medicare-certified, and Joint Commission-accredited, so you can trust that your patients will receive the highest standard of care. We provide:

  • Private Duty (non-medical home services but also medical, licensed by the state of Illinois)
  • Home Health (Medicare-certified, Joint Commission-accredited)
  • Hospice (Medicare-certified, Joint Commission-accredited)

Because we’re an extension of Providence Life Services — a recognized leader in aging services — we have valuable experience in providing care in a variety of settings, including retirement communities, assisted living arrangements, and skilled nursing facilities. We know what it takes to support the systems in place to make sure your staff get the help they need and your clients get the care they deserve.

Our Experience, in Your Setting
Providence can support your residents’ preference for aging in place in their homes on your campus. We have decades of experience that we can bring to your setting, for your staff and your residents.

The “Care Teams” model:
  • Consolidated management structure
  • Centralized intake
  • Centralized billing
  • Consistent quality
  • Cross-trained/shared field staff
  • Continuity in care staff within and across programs
  • Sustained personal relationships with
    patients and clients
Chronic Care Management:

Most seniors over the age of 80 are dealing with three or more chronic illnesses. Treating each disease in isolation ignores the potential interaction of symptoms, medications, and side effects. We provide:

  • Centralized intake
  • Risk stratification and predictive modeling
  • Medication reconciliation
  • Custom-designed care
Coordination of Care meetings:
  • Cross-program support needs
  • Palliative Bridge patients
  • Fragile, at-risk, and/or complex patients
  • Support plans — referrals and resources for patient and family
Patient Education:
  • Customized to the patient
  • Outcome-driven, evidence-based
  • Part of an “Education Pathway”
  • Aligned with Chronic Care Management
  • Communicated with your campus management to fully coordinate care.

Special Programs:
  • Palliative Bridge Program
  • Hospice End-of-Life Care
  • Integrative/Holistic Therapies in Hospice (Massage, Music, Art, and Pet Therapy)

Who benefits from Campus Care? Not just the residents!

Administrators

  • Higher customer satisfaction (your clients receive more personalized attention)
  • Increased occupancy (you can admit a broader range of clients by offering more services)
  • Staff retention (they receive the support they need)
  • Regulatory compliance (staff have time to attend to details)
  • Increased profit margins (more clients stay on-campus longer, aging in place)

Admission Directors

  • Increased census (more prospects qualify for and will be attracted to your expanded services)
  • Increased referral sources (because of all the support offerings and outcome improvement)
  • Better PR (clients and staff satisfaction improves)
  • Greater efficiency (re-hospitalizations and re-admissions are minimized)

Directors of Nursing/Medical Directors

  • Quality patient outcomes (dedicated staff are available for coordinated care)
  • Decreased ER/Urgent Care visits (residents are risk-stratified and care is front-loaded, so re-hospitalization rates are lower)
  • Improved patient family satisfaction (increased support, improved outcomes, and better health)
  • Regulatory compliance (extra support allows staff to attend to details)
  • Support with pain management and end-of-life care (the Hospice Benefit allows this)
  • Help with end-of-life conversations (family support is part of Providence Hospice services)

Social Workers

  • Assistance with discharge planning (our community relations and intake staff understand the paperwork)
  • Support with difficult patients/families (our care staff can ease your caregivers’ workloads)
  • Integrated care planning (we form a web of care between departments)
  • Additional spiritual and psychosocial support (we add our staff resources to yours)
