Skilled nursing in northwest Indiana doesn’t have to look like a nursing home! The “Small House” model of care is a new approach to skilled nursing, rehab, and memory care. Providence Life Services was the first to bring this style of care to Illinois (on our Victorian Village campus), and now we’ve brought it to St. John.

Frank Kray is a long-term resident in a Park Place Small House. He appreciates the life he’s able to have here. “It’s a beautiful place — and beautiful people who take care of you,” he says.

Each Small House building has private rooms for only 10 long-term care people. If you are interested in reserving a place, call to schedule an assessment and begin the application process:

In each Small House, residents have private rooms. Those rooms are positioned to surround an open kitchen/dining area as well as a community living room. Residents are encouraged to participate in the life of the house to whatever extent they are able — helping with housekeeping, working in the garden, making decisions about outings, and more. Such involvement gives people a sense of purpose while exercising cognitive and motor skills.
