After a hospital or rehabilitation stay, you’re sent home. But often, you aren’t quite ready. You need a little extra time and extra help before you feel strong and confident enough to be on your own again. Providence can help with its Transition to Home program.
 
At Transition to Home, our staff can assist with activities of daily living and provide many other services you need. Meanwhile, you will have access to all the amenities our residents enjoy, including three meals a day, activities, and our beautiful grounds. During your entire stay, you can have peace of mind, knowing nursing staff are on site 24/7.
Getting You Ready for Home
We understand you want to go home, and we want to help you get there when you are ready. We’re here to support your goal for a return home. And when you’re ready, our team will work together to give you a helpful plan to follow when you get home. Most people stay less than 30 days in our program to give them the confidence to return home. Here’s what to expect in our Transition to Home program:
PRE-ADMISSION: Trained staff will evaluate your needs based on our assessment, your medical records, and the doctor’s orders. We’ll meet with you and your family to help determine your goals to get you back home.
PLANNING FOR HOME: We will work with you to develop a service plan that meets the needs assessed and will get you closer to your goal of going home. To make sure we are meeting our goals, we will also meet weekly with you and our staff to evaluate progress and make adjustments when needed.
AMENITIES: You will have a furnished private apartment. In addition, you’ll be able to join all our residents for three daily meals, activities, social opportunities, and access to our full community, including our pool. You can take part in our SeniorFit program to help continue to strengthen your body.
What is included in Transition to Home is determined by your individual needs. Some of the assistance we provide includes the following:
• Medications Supplied and Delivered
• Training to properly use Medications on discharge
• Medication Set Up and Reminders (if needed)
• Medication Administration (if needed)
• On-site Labs
• On-site Physician visits available
• Assistance with Activities of Daily Living
• Home Health Care and Therapy from a third-party provider
