A home-centered approach

Skilled nursing care doesn’t have to feel clinical or cold. At Park Place Health & Wellness Center in St. John, Indiana (near Crown Point, IN) we’ve reimagined what skilled nursing can look and feel like. Our innovative “Small House” model de-institutionalizes care, offering a warm, welcoming environment where residents feel truly at home.

Instead of long hallways and sterile rooms, our Small House design feels like a real home. Comfort, dignity, and personal freedom are part of daily life. Take a look at our photo gallery.

What is Small House Skilled Nursing

Unlike traditional nursing homes, our Small House is designed to resemble a private residence. With fewer residents per house and short hallways leading to beautifully designed spaces, it fosters a sense of community and familiarity. Our dedicated care partners take the time to understand each resident's unique needs and interests, allowing them to live on their own schedule while receiving the necessary healthcare oversight.

Our care doesn’t simply meet state regulations, but wins awards. Residents receive an improved quality of life that promotes independence.

Home-like Architecture

Designed for Comfort

A Small House structure is designed like a home, not an institution. In our two skilled nursing buildings, resident rooms surround gracious living areas and a common kitchen. The compact design means the walking distance from bedroom to living room is short enough for most people to navigate without assistance, giving them greater independence.

Flexible Programming

Live on Your Own Schedule

This is not a one-size-fits-all approach. Your schedule is formed around the natural rhythms of your daily life. We recognize that everyone has their own routines and preferences. Whether you're an early riser or a night owl, prefer a morning walk or an afternoon nap, our flexible programming accommodates your natural rhythms. Your freedom is as important to us as your security.

Collaborative Decision-Making

Embracing the motto "Nothing about me without me," we ensure that no decisions about a resident's care are made without their input. Residents are encouraged to participate in daily activities to whatever extent they are able, from assisting with housekeeping to planning events. Meals are prepared in an open kitchen and shared at a large table where staff, residents, and families dine and socialize together, fostering a sense of purpose and community. Resident involvement reinforces a sense of purpose and value while exercising cognitive and motor skills.

“There was so much compassion.”

Lorraine’s health needs had increased from assisted living to skilled nursing care, her daughter, Carol, brought her mother to Park Place, the only place she could trust, part of an organization associated with senior living with over a century. Lorraine enjoyed the personalized care you can only get in a Small House setting. “There was so much compassion,” Carol says.

When Quality Counts, Count on Park Place Skilled Nursing.

Revolutionary Design.

Our unique Skilled Nursing “Small House” at Park Place Health & Wellness Center was thoughtfully designed like a home, centered around the resident. Unlike an institutional nursing home, a typical day’s schedule at the Small House is based on resident preferences, not staff efficiency. The care still meets all state regulations, but it’s highly personalized.

Personalized Attention.

We know nothing is more important than the trust between our residents and their caregivers, so Park Place staff strive to build relationships with our residents. With fewer residents in each of our Skilled Nursing buildings, resident receive the personalized care and attention they deserve.

Holding Ourselves to the Highest Standards.

The Joint Commission is the nation’s oldest and largest standards-setting and accrediting body in healthcare. Park Place has earned the Joint Commission’s prestigious Gold Seal of Approval in Comprehensive Care for Nursing Care Centers. Achieving accreditation is not mandatory for skilled nursing centers, but we appreciate the commitment to quality that The Joint Commission’s standards represent, so we gladly apply.

Caring for Body and Soul.

What truly sets Park Place apart is our faith-based, non-profit mission. Our call is to serve each individual “in the name of Jesus Christ,” and we do that by investing in our staff and our residents, not shareholders. With a chaplain on campus, residents find peace and comfort in our caring environment, regardless of their personal beliefs.

Advanced Clinical Care.

An ounce of prevention is worth a pound of cure. That’s why Park Place has been certified by Vohra Wound Physicians as a Center of Excellence for Wound Management. Our clinical team has undergone specialized training to prevent and treat all types of chronic and complex wound and skin issues, reducing the need to send our residents out to wound care centers or hospitals.

Valuing Our Employees.

We know it takes a special kind of person to provide loving care for seniors, and we value the hard work of our team members through a supportive environment, flexible scheduling, a generous benefits package, and opportunities to advance their careers.
